   (1) - Original signed and notarized 18" x 30" mylar plat for the

               Register of Deeds

   (2) - Mylar copies for County Surveyor & City/Building &

               Planning

   (5) - Blue lines or paper copies

NOTE: Plats must be recorded within 90 days of final approval.

                 If not recorded within 90 days you must present a current letter of

                 extension.
