Welcome to the Passport Division of the Office of the Sarpy County Clerk. Our staff is committed to making the process of securing a passport as efficient as possible. The County Clerk's Office is located in the Administrative area (east side lower level) of the Courthouse.

Application

Completed Form – passport application

DS-11 for New Adult or Minor passports

Proof of Citizenship

Bring 1 of the following:

  • A certified copy (raised seal) of your Birth Certificate (not a hospital copy) with your parent's name listed along with a COPY of your birth certificate is required. 
    • If you were born in Nebraska you can get your birth certificate from the Vital Statistics Office.  Click here for details for the NE Vital Statistics Office.  
    • If you were born outside of Nebraska, you can order a certified copy from www.vitalchek.com or www.vitalrec.com
  • Previously issued passport book or card (for an adult application only)
  • Certificate of Naturalization (adult application only - child application must have birth certificate)

Current Government Issued Photo Identification

  • A "valid" Nebraska Drivers License, State Identification Card, or Military ID along with a COPY of this I.D. 
    • If your Drivers License or Identification card was not issued in Nebraska, please also bring your Social Security card or other form of secondary identification.

Passport Photo

  • Two (2) official Passport Photos (which may be taken at our office - $12.00 fee)
    • Cash, Debit/Credit cards or Money Orders ONLY for the photos. 
      • A debit/credit card merchant fee is collected by a third party processor.
  • No personal checks accepted for Clerk fees

Passport Book ADULT (Age 16 & Older)

  • $110  - Routine Service 4-6 weeks
  • $185.89 - Expedited Service 2-3 weeks

Passport Book MINOR(Under Age 16) ** Requires both parents to be present or the absent parent to complete a DS-3053

  • $80  - Routine Service 4-6 weeks
  • $155.89 - Expedited Service 2-3 weeks

Passport Card ADULT (Age 16 & Older)

  • $30 - Routine Service 4-6 weeks
  • $90 - Expedited Service 2-3 weeks (no overnight delivery return mail available) 

Passport Card MINOR (Under Age 16) Requires both parents to be present or  the absent parent to complete a DS-3053

  • $15 - Routine Service 4-6 weeks
  • $75 - Expedited Service 2-3 weeks (no overnight delivery return mail available) 

Processing Fee

  • $35 Processing fee for EACH application submitted is paid to the County Clerk's office
    • Cash, Debit/Credit cards or Money Orders ONLY for the execution fee  
      • A debit/credit card merchant fee is collected by a third party processor.
  • No personal checks accepted for Clerk fees

General Passport Information

A passport BOOK allows to you to travel to many more locations via many more modes of transportation than the passport card alone. It is possible for you to apply for or have both the passport card and the passport book, although not necessary.

The passport CARD facilitates entry and expedites document processing at U.S. land and sea ports-of-entry when arriving from Canada, Mexico, the Caribbean and Bermuda. The card may not be used to travel by air. Otherwise, it carries the rights and privileges of the U.S. passport book and is adjudicated to the exact same standards.

A Passport is valid for 10 years for persons 16 years of age and older and valid for 5 years for minors under 16 years of age.

Children under 16 years of age must have BOTH parents or legal guardians with them when applying for a Passport.  16-17 year olds must have ONE parent with them when applying.

Passport Book or card MINOR under age 16 are NOT eligible for renewals.  Parents and child must apply in person at the County Clerk's office.  See "Passport Application" link at the top of the page.

Passport Processing time after submission through the Clerk's office

A passport will not be issued immediately at the Clerk’s office.  The completed application will be sent from the Clerk’s Office to the Passport Services in the U.S. Department of State.

Routine passport application may take 4-6 weeks from the date of remittance.  This should be taken into consideration as travel plans are made.  For the most up-to-date processing times, please refer to http://www.travel.state.gov/passport/processing/processing_1740.html

Expedited applications have been processed and returned to the applicant in as little as 2-3 weeks.

*The District Court Clerk’s Office, located in the building on the west side (Court side) accepts passport applications from 8 a.m. to 2 p.m. weekdays.
